The Assertive Outreach Team's work focuses upon people with a severe and enduring mental health problems in the borough of Hillingdon, who have difficulty accessing traditional or mainstream mental health services. There is often a history of repeated hospital admissions. The team accepts referrals from the community mental health teams when an intensive and flexible approach is required to maintain engagement with the service user. The team’s key tasks are to provide ongoing assessment, care co-ordination and a range of medical, psychological and social interventions that are based up on the evidence of best practice. To achieve this, working closely with friends, families and communities is often required.
Due to the complexity of the assertive outreach team's service users' ’needs, team members fulfil a number of statutory duties requiring them to work in close partnership with other health, social and criminal justice agencies.
