Service Description

Rehabilitation and Supporting People ServiceThe Rehabilitation and Supporting People Service is a multi-disciplinary service providing daily support to clients from an administrative base, the Resource Centre and a staffed hostel. The hostel team provide a 24 hour emergency cover to all the clients whose care is managed by the service. The support services are broadly organised across three teams of operational staff: the Resource Centre team, the hostel team and the specialist link team. In practice, the teams co-operate closely in working to rehabilitate and support clients.

All clients have a named Care Manager within the service and in addition may be allocated a specified Keyworker for supplementary structured support. The Care Manager is responsible for organising a needs-led care package for the client from either within the service resources or utilising the resources of other outside agencies where necessary.

Each client has a full multidisciplinary care review every six months using a specially developed holistic assessment format called the Needs-for-Care-Assessment. This involves reviewing all aspects of the clients' social, emotional and physical needs. Clients are able to attend these reviews and any recommendations that arise from the review are discussed with the client.

Referrals are received from Park Royal Centre for Mental Health and from the Brent community mental health teams. All referrals are processed by the project Approved Social Worker.
