Central Kensington CMHT previously known as Courtfield CMHT provides a service to people living within the Earls Court and Kensington area who have a severe / enduring mental illness and who are under the care of the South Kensington and Chelsea Mental Health Centre.
The team comprises of a manager; two administrators; a Senior Community Psychiatric Nurse, three Community Psychiatric Nurses (CPNs), two Primary Care Liaison Nurses; one Senior Social Worker; five social workers and an occupational therapist. The service is open Monday - Thursday 9-5pm and Friday 9-4.45pm.
We work with two consultant psychiatrists at South Kensington and Chelsea Mental Health Centre. Their patients will be registered with GPs based in the SW5, SW7, W8 and W14 areas of the borough. We also work with people who are not registered with GPs but who are living in the area.
We work with people who are subject to the Care Programme Approach, providing an assessment of their needs in collaboration with the client and their carer, if applicable. Following their assessment, which could involve other professionals in the service, a care plan will be devised which will be monitored and reviewed by the main worker involved (Care Co-ordinator).
We also worked with people who are in-patients of the acute psychiatric ward at South Kensington and Chelsea Mental Health Centre, working towards discharge and planning their follow-up care. Sometimes people may need to move to alternative accommodation, with varying levels of support from daily visits to 24-hour support.
