The Crisis Resolution and A&E Liaison Team provides assessment and home treatment services for adults of working age living in the borough of Hillingdon, who suffer from severe and enduring mental health problems and require intensive care in their home to avoid hospital admission. The Crisis Resolution and A&E Liaison Team has a gatekeeper function, which means that all request for admissions to inpatient care must be discussed with the team so their user and carer can be offered an alternative package of care in their own home if appropriate.
The Crisis Resolution and A&E Liaison Team also provides an assessment for individuals who present at Hillingdon Hospital’s Accident & Emergency department. Although the team targets those with severe and enduring mental illness, it will also take on people for home treatment who do not fall into that category, because they would be admitted to the ward if the team did not intervene. If admission is necessary, the Crisis Resolution and A&E Liaison Team can assist the individual for early discharge from inpatient care and offer support/treatment at home and co-ordinate follow-up care with the relevant community mental health team.
The service is open 24 hours a day and 365 day of the year. Patients have to be referred to the Crisis Resolution and A&E Liaison Teamby a mental health professional.
