The Community Mental Health Team (CMHT) A is a multidisciplinary team based in North Kensington. The areas covered are W10 and parts of W11. The team consists of an Administrator, a Team Manager, Approved Social Workers/Care Managers, Community Psychiatric Nurses (CPNs), a Consultant Psychiatrist, a Senior Registrar, a Staff Grade Psychiatrist and a Junior Doctor. There is a Psychologist and a Family Therapist who provide sessional work for the Team.
The Service has links with other local teams such as the Day Hospitals, inpatient wards, Rapid Response and Home Treatment Team, the Opendoor Community Support Team, West London Cyrenians and Richmond Fellowship.
The CMHT provides mental health assessment, case management and ongoing support to clients between the ages of 16-65 years, as well as support for their carers. The Team uses an Integrated Care Programme Approach and Care Management model of care within the multidisciplinary team set-up.
The CMHT currently operates between 9-5pm, and the Assertive Outreach Service provides extended service on weekends.
The Team operates a rotational duty system together with the other two CMHTs in the area CMHT "B" & CMHT "C"). There is an open access referral system where service users can self-refer 24 hours a day. Friends, relatives, carers, professionals and other agencies can also refer to the service for a specialist mental health assessment.
