The Community Mental Health Team (CMHT) B is an inter-agency, multi-disciplinary team based in North Kensington. The areas covered are W10 and parts of W11. The team consists of an Administrator, a Team Manager, Approved Social Workers/Care Managers, Community Psychiatric Nurses (CPNs), Consultant Psychiatrist, Senior Registrar, Staff Grade Psychiatrist and a Junior Doctor. There is a Psychologist and Family Therapist who provide sessional work for the team.
The CMHT provides mental health assessment, case management and ongoing support to clients between the ages of 16-65 and support for the carers. The team uses an Integrated Care Programme Approach and Care Management model of care within the inter-agency, multi-disciplinary team set-up.
The CMHT currently operates between 9-5pm, and the Assertive Outreach Service provides extended service on weekends to an identified caseload.
The Team operates a rotational duty system together with the other two CMHTs in the area CMHT "A" & CMHT "C"). There is an open access referral system where service users can self-refer 24 hours a day. Friends, relatives, carers, professionals and agencies can also refer to the service for a specialist mental health assessment. All three teams are sectorising with local GPs.
