This is one of the three Community Mental Health Teams in North Kensington. We serve residents of the Royal Borough of Kensington and Chelsea who suffer from severe and enduring mental health problems.
The Team consists of four Social Workers, three Community Psychiatric Nurses, one Primary Care Liaison Nurse, Consultant Psychiatrist, two junior doctors and a Consultant Psychologist. The Team benefits from weekly input from a Social Worker from the Parental Mental Health Service. Offering assessments of needs and care planning under the combined CPA/Care Management model, the Team aims to work in close collaboration with service users and their carers.
