The community mental health team offers a variety of services to adults of working age in Uxbridge and West Drayton (who have severe and enduring mental health problems), their families and carers.
The team’s key tasks are to provide assessments, care co-ordination and a range of medical, psychological and social interventions that are based upon the evidence of best practice. Most referrals to the team come by way of primary care. We aim to offer a rapid assessment service. Where people do not meet the criteria or secondary mental health care, they will be directed to another more appropriate service and their referrer will be informed.
The team comprises a mix of mental health practitioners including psychiatrists, approved social workers, nurses, occupational therapists and psychologists. A carers’ support worker, independent advocacy and welfare benefits advice complements this multi-disciplinary approach. We also work in partnership with other statutory and independent agencies.
