The Day Hospital provides assessment, therapeutic treatment, rehabilitation and respite care to people over 65 years of age. It can cater for up to thirty clients a day. The clients are from a diverse population in North Kensington and Westminster. The team is multi-disciplinary (MDT) and able to provide a wide range of therapies and assessments, utilising a holistic approach.
Specific days are allocated for clients depending on their assessed needs and mental health problems along with the reason for referral:
Monday & Friday - For short to mid-term assessment and / or treatment of those with a functional mental health problem.
Tuesday –For the assessment and therapeutic treatment for those clients in the very early stages of Dementia.
Wednesday - For the assessment and therapeutic treatment of those clients with a moderate or severe cognitive impairment
Thursday - This day is for clients who need long term input, for those who are particularly vulnerable, requiring close monitoring while living in the community. These clients have already been assessed at the day hospital.
Chamberlain Day Hospital’s multi disciplinary team includes nurses, occupational therapists, art and music therapists. It is supported by The Senior Day Hospital by other members of the MDT including: doctors, psychologists, a family therapist, social workers, CPNs, admiral nurses and a dietician.
Chamberlain Day Hospital operates Monday –Friday from 8.30 am –4.30pm. It is closed on public holidays. All new clients referred must come via the Community Assessment Team, at St Charles Hospital.
