We are a small team of specialist Community mental Health Nurses providing treatment for people aged 65 and over, and living in the North Kensington and Paddington area of Westminster. The Service aims to:
- Promote the wellbeing of the older people with mental health problems.
- Provide home and outpatient based assessment for users of the service.
- Coordinate and deliver care to people with functional or organic mental health problems.
- Provide effective treatment when necessary.
- Promote good joint Working Practice with local services and our existing links with local authorities.
Referrals can be made by Community Assessment Team (CAT), GPs, Supporting services in Residential Homes and Day Centres, Brent Admiral Nurses and Social Services.
The Service is open Monday - Friday 9-5, flexible.
