The team of nursing staff provides intensive assessment, treatment and support for older people with acute mental health problems for up to 12 weeks, however this can be extended. They will provide care in the home as an alternative to admission or intensive support to enable early discharge from hospital to home.
A key role of the team is to monitor and assess needs to ensure adequate care input to support the person at home. In order to provide this, they work closely with Carers, Social Services, GPs, District Nurses and other services. They will also provide education and support to other care staff. This service is only accessible via a GP who refers through the psychiatrist.
