The community mental health team offers a variety of services to adults of working age living in Northwood, Ruislip and Harefield who have severe and enduring mental health problems, their families and carers.
The team’s key tasks are to provide assessments, care co-ordination and a range of medical, psychological and social interventions that are based upon evidence of best practice. Most referrals to the team come by way of primary care. We aim to offer a rapid assessment service. Where people do not meet the criteria for secondary mental health care, they will be signposted to another more appropriate service and their referrer will be informed.
The team comprises a mix of mental health practitioners including psychiatrists, approved social workers, nurses, occupational therapists and psychologists. Carers assessment and support is offered through allocated care coordinators. We have access to a drug and alcohol specialist nurse on a sessional basis. We also work in partnership with other statutory and independent agencies.
