Service DescriptionIt is commonly known that pharmacists play an integral part in the delivery of health promotion and the safe monitoring/dispensing of prescriptions whilst also building a good rapport with their clients.
This scheme wishes to recognise and enhance the day to day work of the pharmacist with their substance using clients.
The aim of the scheme is to improve the health of substance users by developing a shared care scheme, based within pharmacies.
Our objectives are:
- To improve the consistency and quality of care to the client.
- To ensure, as far as possible, that methadone prescribed is used only by the client.
- To minimise the risk of harm to the client and to others.
- To improve communication between pharmacists, GPs, their clients and other named health professionals.
The SMMCP Scheme incorporates two levels of involvement
1. Core Services:
The 'Core Services' include an agreement by Pharmacist, GP and client to work together identifying any 'concerns' before they become 'problems' in order that they may be resolved as quickly and safely as possible ie missed doses of medication, unacceptable behaviour, health concerns, and intoxication.
2. Additional Services:
- the installment dispensing of benzodiazepines, tri-cyclic antidepressants,
anti-psychotics and buprenorphine.
- the supervised consumption of methadone mixture and buprenorphine
