Service Description
The Central and North West London (CNWL) Mental Health NHS Trust, Substance Misuse Service, Hammersmith and Fulham Drug Treatment Centre aims to provide a range of specialist health care interventions for individuals and their families who are affected by the misuse of primarily opiate drugs within the Hammersmith and Fulham Borough.  We provide short and long term treatment for opiate users, through a programme of structured phase of care.  This also includes on-site dispensing of oral medication and range of therapeutic interventions.

Your Referral
Referral to the Hammersmith and Fulham DTC is via the Community Assessment and Primary Services (CAPS), which is located in the same building.  Clients will be offered a confidential assessment to ensure they meet the admission criteria.

Your First Appointment
You will be allocated a keyworker who is a clinical member of staff who will assess you and draw up a treatment plan to meet your individual needs.  Once agreed, the plan is considered to be part of a treatment agreement with the Hammersmith and Fulham DTC which you will be expected to agree and sign as part of the first phase of your treatment.

Future Appointments
These will be arranged by your keyworker who is a Clinical Nurse Specialist. Frequency of attendance is dependant upon the phase of treatment you are in and your response to the level of care offered.

Access to Treatment Restrictions 
Persons who are under 16 years of age will not be treated within the service.  Persons who do not reside or have a GP within the Hammersmith & Fulham Borough will not be treated within the service.

Groups
In addition to individual counselling and support with your keyworker, a range of therapeutic groups are available.

  • Stress Management Group –Aims to offer support and education.  It also allows you to share experiences of stress, anxiety, and also teaches you how to cope with these situations through various relaxation techniques.

  • Creative Groups –Provides opportunities to express yourself and develop new interests in an informal and social setting e.g. Batik and Photography. Groups are based at the clinic or in community venues in conjunction with a local arts organisation.

  • Gardening Project –This enables you to experience the benefits of cultivating a garden: growing your own flowers and vegetables, developing new skills and knowledge in a relaxing and supportive environment.

Other Services 
The therapeutic programme aims to provide a comprehensive range of therapies, activities, advice and group sessions. These include psychological treatment, family therapy, health education groups and occupational therapies.

Liaison with Social Services is available where appropriate and referrals can be made for vocational guidance.

Confidentiality
Your treatment files are confidential to CNWL’s Substance Misuse Service and will not leave the building without your permission, unless you transfer to another treatment unit within the Substance Misuse Service.  In most cases, we cannot disclose or obtain information about you without your written consent.  In addition, you have the right to request sight of your treatment files.  Please see your keyworker for further information.

Complaints 
Many complaints can be resolved immediately by discussing your complaint with a member of staff.  However, if you are still not satisfied, ask the staff member to help you fill in an official complaint recording form.  Alternatively, you can write in with a confidential complaint, which will be processed through a formal Complaints Procedure.  Information about the Complaints Procedure is available on request.  In addition, instructions on how to make a complaint are clearly displayed in client waiting areas.

PALS –(Patient Advice and Liaison Service)
Each service in the Trust has its own PALS link worker, who is a member of CNWL staff.  The Link worker can advise you on how to complain if you want to and put you in contact with advocates.

Additional Services 
With the exception of the Thursday afternoon Women Only Session, there are no child care facilities available and therefore parents are requested to be responsible for their children at all times.

Travelling Expenses 
The process will be explained to you when attending the service. Fares will be reimbursed provided:

a) You receive income support or family credit.
b) You have a specific appointment.
c) You have a valid London Transport ticket showing the cost, date and time of travel.
